Salicylic acid is an exfoliating ingredient used in topical formulations for blocked pores, blackheads and blemish-prone skin. This page explains its action, formulation types, evidence and principal precautions. It is not a guide to selecting an individual acne treatment.

What is salicylic acid?

Salicylic acid is commonly described in skincare as a beta hydroxy acid, or BHA. It is lipophilic, meaning it has an affinity for oily environments, and helps accumulated cells shed from the outer skin layer.

It is found in rinse-off cleansers, leave-on liquids and pre-saturated pads. Different formulations can contain additional exfoliating or supporting ingredients, so the presence of salicylic acid alone does not establish how a product will behave.

Salicylic acid also appears in preparations for other skin conditions. Products intended for warts, corns or other areas of thickened skin are not interchangeable with facial skincare.

How does salicylic acid work?

Salicylic acid is often described as keratolytic. Its desmolytic action helps reduce adhesion between cells in the outer skin layer, allowing accumulated cells to shed more readily.

This action is relevant to follicular congestion. When cells and sebum accumulate within a hair follicle, they can contribute to the formation of a comedone. Open comedones are commonly called blackheads; closed comedones are commonly called whiteheads.

The response to a salicylic-acid formulation depends on several factors:

  • The nature and severity of the skin concern
  • The concentration and formulation vehicle
  • Whether the preparation is rinsed away or left on
  • Application amount and frequency
  • Other exfoliating ingredients or treatments
  • Skin sensitivity and barrier condition

Blackheads are not evidence of dirty skin. Aggressive washing or scrubbing can increase irritation without resolving the underlying tendency to develop blocked follicles.

Clinical relevance of salicylic acid

Blackheads and whiteheads

Salicylic acid is relevant to comedonal congestion because it helps loosen accumulated cells involved in follicular blockage. It cannot permanently prevent every comedone from forming.

Oily and congested skin

Salicylic acid is frequently incorporated into formulations for oily skin. Removing excess surface oil should be distinguished from permanently suppressing sebum production. An ingredient listing does not establish that a formulation will reduce oil production.

Blemish-prone skin

Acne involves several processes, including follicular blockage, sebum, inflammation and microbial activity. Salicylic acid addresses part of this picture and is not a complete treatment for every form of acne.

Painful nodules, cysts, scarring or persistent inflammatory acne require appropriate assessment. Increasing exfoliation can add irritation without addressing the treatment needed.

Uneven surface texture

Exfoliation can affect roughness associated with accumulated surface cells. This should not be confused with correcting established acne scars or treating every cause of uneven skin texture.

Clinical evidence and its limitations

The 2024 American Academy of Dermatology acne guideline includes topical salicylic acid as a conditional recommendation. This recognises it as an option in appropriate circumstances, with treatment decisions depending on the available evidence, individual needs and tolerability.

Experimental research has investigated effects on lipid production and inflammatory signalling in sebocyte cells. Laboratory findings help explain possible mechanisms but do not establish the degree of improvement obtainable from a particular finished skincare product.

Professional salicylic-acid peels involve different concentrations and application conditions from routine home skincare. Their reported outcomes should not be transferred directly to a cleanser, toner or pad.

Salicylic acid concentrations

The percentage describes the amount of salicylic acid within a formulation. It does not, by itself, determine suitability or clinical performance.

A rinse-off cleanser and a leave-on liquid containing the same percentage expose the skin differently. Vehicle, pH, accompanying ingredients and application frequency also matter.

A higher percentage should not automatically be interpreted as a better choice. Equally, a formulation without a prominently stated percentage cannot be ranked reliably from its ingredient list alone.

Salicylic acid cleansers, liquids and pads

Rinse-off cleansers

A cleanser introduces salicylic acid during washing and is then rinsed away. Its cleansing agents also influence skin feel and tolerance. Extending contact time beyond the supplied directions is not a reliable way to improve its effect.

Leave-on liquids and toners

Leave-on formulations remain on the skin after application. An exfoliating toner has a different function from a simple hydrating toner, even when both occupy the same position after cleansing.

Pre-saturated pads

Pads provide a method of applying a liquid formulation. Some contain several exfoliating ingredients. The liquid and the friction from wiping both contribute to the experience, and repeated passes may increase irritation.

Using an exfoliating cleanser and a leave-on exfoliant together increases total exposure. A routine does not necessarily require both.

Skin tolerance and ongoing review

The appropriate frequency depends on the individual formulation and the complete routine. Follow the supplied directions rather than applying one schedule to every salicylic-acid preparation.

Relevant considerations include:

  • Whether the skin remains comfortable between applications
  • Whether dryness or persistent stinging develops
  • Other acids, scrubs or acne treatments already being used
  • Whether the original concern is improving or needs reassessment
  • Whether application frequency should be reduced or use stopped

Oily skin can also be dehydrated or irritated. Tightness and discomfort should not be treated as evidence of more effective cleansing.

Principal safety considerations

Salicylic-acid formulations can cause dryness, stinging, redness and peeling. Persistent irritation is not proof that a formulation is working.

  • Follow the stated amount, application area and frequency.
  • Avoid contact with eyes, lips and mucous membranes.
  • Do not apply to broken or markedly irritated skin.
  • Avoid unnecessary combinations of exfoliating products.
  • Use a suitable moisturiser where needed for comfort.
  • Reduce use or stop if persistent burning or significant irritation develops.
  • Obtain advice for severe, unexpected or continuing reactions.

Known salicylate allergy, pregnancy, breastfeeding and intended application over large areas should be discussed with an appropriate healthcare professional. Follow any first-use testing instructions provided with the formulation.

Salicylic acid and other skincare ingredients

Some finished formulations combine salicylic acid with other exfoliating acids. Layering several separate products does not reproduce that formulation and may increase irritation.

Other acne treatments can also cause dryness. Anyone following a prescribed regimen should check with the treating clinician before introducing another exfoliating step.

A moisturiser can support comfort but does not make excessive exfoliation harmless. The complete routine should be assessed when irritation develops.

Salicylic acid and sun protection

Daily sun protection remains relevant, particularly when post-inflammatory pigmentation is a concern. Follow the sun-exposure precautions supplied with the individual product, including any precautions related to other acids in the formulation.
